Welcome to the ESTRID questionnaire

If you have been invited to the ESTRID study, we have sent a paper questionnaire as well as login information for the web-based questionnaire, if you would rather answer digitally. To log in and answer the questionnaire digitally, click here. You need your username and password that we have sent to you. If you do not have them, please contact us and we will be able to help you. Your answers are saved automatically and you can log out at any time and continue to answer the questionnaire at a later time.

If you prefer to answer by filling in the questionnaire in paper format, fill it out as accurately as possible and then fill out the consent form (which is also included) and then mail these two back to us using the reply envelope. The postage is already paid, so no stamp is needed.

It is the same questionnaire that can be answered in two different ways, to make it as easy as possible for everyone invited to participate in the way that suits them best. For those who wish, there is the possibility to complete a version of the questionnaire in Arabic or Serbo-Croatian.

The more people participate in the study, the better the results. We are therefore very grateful that you want to participate!
