Nutrient deficiencies still occur in pregnant women and young children in Sweden and are prevalent for some nutrients in certain risk groups. Cognitive and behavioral problems are common in children and early nutrition is a causal factor of public health importance. We will determine in a series of randomized, controlled clinical trials whether nutritional interventions during different developmental periods improve later cognitive and behavioural test scores:a) Iron supplementation of healthy infants who are breastfed &gt
    4 months (n=228), evaluated at 1-3 years
    b) Iodine supplementation of pregnant women (n=1275), children evaluated at 3 yearsc) Hydrolysed cow’s milk formula given to healthy infants (n=234), evaluated at 1 year
    d) Long-chain polyunsaturated fatty acid supplementation of preterm infants (n=129), evaluated at 2 years. We will determine in a birth cohort (n=10000) as well as in several population-based cohorts of very preterm infants how early nutrition/diet and biomarkers of nutrition are associated with cognitive and behavioral development up to (pre-)school age.For assessment of neurodevelopmental risk, we use state-of the art methods (brain MRI, psychological tests) and we develop novel tools e.g. functional near infrared spectroscopy and neurofilament light. This project will give the evidence base for future national and international recommendations on early life nutrition and may help reduce the prevalence of cognitive and behavioral problems in children.

The purpose of our research is to improve the health of premature babies and their families in Canada and around the world. Our approach is to analyze information collected from thousands of babies in hundreds of hospital units in Canada and several countries or regions in Europe, Asia, and Australia. Information about the health and treatment of premature babies is contributed by "neonatal networks" representing hospitals in each of 9 regions. This cooperative group of networks is called the International Network for Evaluating Outcomes of Neonates (iNeo), and we have been working together for the past 7 years. Our first step was to look for regional similarities and differences in healthcare practices and medical outcomes for premature babies and identify areas of success and areas where outcomes could be improved. Now, our next step will be to compare data from individual premature babies across the more than 350 hospital units in our worldwide network, looking to see how differences in care practices affect the babies' medical and developmental outcomes - both as newborns and as young children. Knowledge gained may help us to study beneficial intervention further or develop implementation science projects. Our research team brings together healthcare providers, health system managers, parents, and policy makers from across the world who are united in their goal to improve the care and outcomes of premature babies. Together we will continue building our valuable data sets with more information and new variables, and we will enrich our collaboration by engaging networks in low-or-middle-income countries. Importantly, our project partners will include national, parent-run pediatric advocacy organizations whose expertise will help focus our research priorities and communicate our results effectively to interested Canadians.
