Jag är anknyten som forskare vid Avdelningen för omvårdnad och arbetar som universitetssjuksköterska samt enhetschef vid Hjärtkliniken, Danderyds sjukhus. Min forskning fokuserar på utveckling, utvärdering och implementering av komplexa vårdinterventioner för personer med långvariga sjukdomar och deras närstående. Jag är projektledare för det nationella forskningsprojektet Stepstones-Implement, som syftar till att implementera och utvärdera ett evidensbaserat och personcentrerat övergångsprogram för ungdomar med långvariga sjukdomstillstånd i övergången till vuxensjukvården.

Stepstones-Implement
Huvudansvarig forskare – i samarbete med Göteborgs universitet och sex universitetssjukhus i Sverige
Syftet med Stepstones-Implement är att implementera ett effektivt och hållbart personcentrerat övergångsprogram för ungdomar med hjärtsjukdomar och andra långvariga sjukdomstillstånd. Projektet har tre huvudsakliga mål:- Identifiera främjande faktorer och hinder för implementeringen av ett personcentrerat övergångsprogram vid sex svenska universitetssjukhus som erbjuder specialiserad vård för ungdomar med hjärtfel.
- Implementera programmet med fördefinierade strategier och utvärdera dess effekter på patientrapporterade utfall (t.ex. egenmakt, förberedelse inför övergång, livskvalitet och vårdupplevelse) samt attityder till övergångsvård bland hälso- och sjukvårdspersonal.
- Undersöka betydelsen av lokala, särskilt utbildade facilitatorer för implementering och långsiktig hållbarhet.
Stepstones – Neuropsykiatriska tillstånd
Huvudansvarig forskare – i samarbete med Göteborgs universitet och Västra Götalandsregionen
Projektets syfte är att anpassa och samskapa ett personcentrerat övergångsprogram för ungdomar med neuropsykiatriska tillstånd i övergången till vuxenlivet och vuxensjukvård. Projektet har två mål:- Utforska hur STEPSTONES-programmet kan anpassas genom intervjuer med ungdomar, föräldrar och hälso- och sjukvårdspersonal.
- Samskapa och anpassa programmet med hjälp av erfarenhetsbaserad samskapande metodik (experience-based co-design).
Digi-Stepstones
Medforskare – i samarbete med Göteborgs universitet och Västra Götalandsregionen
Digi-STEPSTONES syftar till att utveckla och utvärdera ett digitalt, personcentrerat övergångsprogram som stödjer ungdomar med långvariga sjukdomar att bli aktiva deltagare i sin egen hälsa och vård under övergången till vuxenlivet. Interventionen bygger på det ursprungliga Stepstones-programmet och omfattar tre planerade delstudier.PeriBrainCare – Postoperativ kognitiv återhämtning
Medforskare – i samarbete med Karolinska Institutet, Västra Götalandsregionen och Region Stockholm
Projektet undersöker postoperativ neurokognitiv återhämtning hos äldre personer som genomgår kirurgi. Det använder en bred metodologisk ansats, inklusive analoga och digitala neurokognitiva tester, biomarkörer, patient- och närståendes erfarenheter, symtomkartläggning samt longitudinell uppföljning för att förstå återhämtningsförlopp och relaterade faktorer.Trach-Inspire

- Swedish Research Council for Health Working Life and Welfare1 February 2025 - 31 January 2028Young people with long-term conditions need to be transferred from pediatric to adult healthcare to ensure continuous medical follow-up and health throughout their lives. Transition programs have been developed to empower young people to take charge of their health and care as they transition into adulthood. These programs have the potential to reduce the risk of complications and positively impact overall health and quality of life. Internationally, the interest in this research field has increased in the past decade. In Sweden, transition programs have been evaluated through several research projects that combine various methodologies and have involved different healthcare settings. Considering the ongoing developments in this field, as well as the strong interest from healthcare providers and the research community, the need for a national research network for transitional care in Sweden has become evident. The purpose of the research network is to strengthen research in this area, coordinate research initiatives, and implement research findings in the surrounding community. The network will not only work closely with researchers, but will include the target groups, i.e., young people with long-term conditions, their parents, healthcare professionals and representatives from interest and patient organizations, in the planned activities. This close collaboration will ensure implementation strategies and future research projects are aligned with the needs of the population. The steering committee (i.e. the researchers in this application) come from several Swedish universities and colleges, with different educational backgrounds and at different stages in their academic and research career. All members are involved in ongoing and planned research projects related to transitional care.
- Swedish Research Council1 January 2025 - 31 December 2027The number of older people who undergo complex surgical procedures continues to rise. Old age is a significant risk factor for the development of postoperative neurocognitive decline which can lead to problems with performing daily activities and maintaining independence. The purpose of this study is to evaluate if the PeriBrainCare bundle has a positive effect on older people’s postoperative cognitive recovery after cardiac surgery. The aim is, to test if the PeriBrainCare bundle reduces postoperative cognitive complaints as reflected in postoperative symptoms and symptoms burden.This intervention will be nested in an ongoing trial performed by the PeriBrainCare group 2025-2029. A pragmatic stepwise cluster randomized study with data collection at two Thoracic clinics in Sweden, 220 participants ≥65 years old with a relative will be included. The PeriBrainCare bundle is based on international recommendations, guidelines, and research and contains three components: Preoperative patient education, a postoperative follow-up, and evaluating cognitive symptoms and complaints. Primary outcome is neurocognitive complaints, secondary outcomes are factors associated with cognitive decline, delirium, depression, functioning in daily life, frailty, and health economics. Patient´s and next of kin´s experiences will be assessed and evaluated. The results will have immediate relevance for a substantial number of older persons undergoing surgery, and their close relatives.
- Swedish Research Council for Health Working Life and Welfare1 January 2024 - 31 December 2026Research problem and specific questionsAs adolescents with special healthcare needs must transition from pediatric to adult care, transition programs are needed and advocated. Studies evaluating the efficacy of these programs have been performed but no study has evaluated the effectiveness of a transition program in a real-life scenario. Thus, the purpose of this project is to implement an effective and sustainable person-centered transition program for adolescents with special healthcare needs in Swedish healthcare.Data and MethodThree studies will be performed(i) an implementation study using predefined implementation strategies at university hospitals in Sweden providing specialized care for adolescents with special healthcare needs and evaluating the effects in patient-reported outcomes (empowerment, transition readiness, disease-related knowledge and care experience) compared to a historical cohort from a previous randomized controlled trial(ii) a process evaluation of the implementation of the program and strategies used to facilitate implementation at the participating sites.and (iii) a needs assessment exploring the prerequisites for and adaptation of the transition program for adolescents with developmental and neuropsychiatric disabilities.Societal relevance and utilisationThe transition of care for adolescents with special healthcare needs must be more person-centered and tailored to the individual young persons and families needs. The project will provide evidence on the effectiveness of a person-centered transition program and which strategies are most successful in achieving effective and sustainable implementation of person-centered care interventions for adolescents in transition to adulthood. Furthermore, how transition programs can be adapted to the special needs of adolescents with disabilities is pivotal in providing equity in healthcare for diverse populations.Plan for project realisationThe transition program will be implemented and studied at specialized children´s wards at six university hospitals in Sweden during 2024.Study 1 and 2 will be carried out during 2024-2027 and study 3 during 2024-2025.
- Swedish Research Council1 January 2024 - 31 December 2026The society is ageing and advantages in anesthesia and surgery allows more complex interventions in older persons. Old age is a significant risk factors for the development of postoperative neurocognitive decline characterized by a gradual decrease in performance in several cognitive domains such as memory, attention, information processing and executive functions, leading to problems with performing daily activities and maintaining independence and postoperative complications. The purpose is to measure older persons’ postoperative neurocognitive function, to detect neurocognitive decline, and to identify risk factors and difficulties in daily living as well as explore close relatives’ experiences of it.A longitudinal observational mixed method study including 200 participants ≥65 years of age undergoing planned cardiac surgery in two different Thoracic surgery departments. Neurocognitive function will be assessed with a digital neurocognitive test battery, seven times over three years postoperatively. Risk/affected factors such as delirium, functional status, recovery, health literacy depression, unplanned healthcare visits and frailty as well as patients and close relative’s symptom burden experiences will be measured.The results will have immediate relevance for a substantial number of older persons undergoing surgery, and their close relatives, by enhanced knowledge about postoperative cognitive decline/recovery and support to be implemented
