Forskningsbeskrivning

  • Min forskargrupp har 2 huvudspår i forskningen. Vi undersöker olika aspekter av det farliga samspelet mellan njursjukdomar och utvecklingen av hjärtkärl-sjukdomar i befolkningen. Vårt nuvarande huvudprojekt fokuserar på betydelsen av vår tarm- och urin-flora för uppkomsten av njur- och hjärtkärl-sjukdomar. Vi undersöker även sambandet mellan kronisk smärta och utvecklingen av hjärtkärlsjukdomar i ett separat stort projekt.

    The overall hypothesis of this proposal is that chronic pain is a clinically relevant contributor to the development of cardiovascular disease in the community. Importantly, previous studies reporting associations between chronic pain and cardiovascular disease have been limited by small study samples, cross-sectional study design, lack of data on specific cardiovascular events and risk factors, limited data on pain severity and psychosocial factors, risk of reverse causation, and lack of sex-specific analyses.Therefore, we aim to comprehensively investigate the detrimental interplay between chronic pain and cardiovascular disease using multiple large epidemiological study cohorts and cohorts with real world clinical data (total n&gt
    1 000 000). The different study samples have detailed data on pain phenotypes, psychosocial factors, treatments, co-morbidities and incidence of cardiovascular disease and mortality, but also large-scale data on metabolomics and proteomics which can provide additional insights to underlying mechanisms. If successful, we expect that our findings will lead to important new insights of the interplay between chronic pain and cardiovascular disease that may have implications for the routine care of chronic pain patients in primary care. We hope to establish that patients with chronic pain in primary care are at substantially increased cardiovascular risk and would benefit from an intensified preventive treatment.

    Research problem Chronic pain is a leading cause of disability worldwide with huge impact on public health and welfare systems. Interdisciplinary treatment (IDT) is currently the established treatment in Swedish specialist care, but recent research shows that treatment effects are small or non-existent. It is recognised that both selection processes and individualised treatment need improvement! With the purpose to enhance the effectiveness of Swedish specialised IDT in patients with chronic pain we plan for three Objectives:To develop an intelligent Clinical Decision Support System for use in treatment individualisation of specialised IDT and validate its design, performance, and feasibility.To prospectively evaluate the system’s clinical effectiveness and cost-utility in a registry-based clustered-randomised control trial (RRCT).To implement the support system in Swedish specialised IDT. The system will facilitate patient selection and guide context-specific individualised treatment strategies by systematically learning patient-specific patterns from big-data. Data and methodWe will apply artificial intelligence (AI) methods to clinical data from over 60,000 patients with chronic pain across 40 specialist units in Sweden for the period 2009-2022 (the FRIDA-database). FRIDA is today one of the world´s largest rehabilitation databases that synchronises unique and longitudinal data from the National Register of Pain Rehabilitation with four other national registers. The project will be carried out in close collaboration with healthcare providers, patient organisations, policymakers and the business community. Plan for project realisationOur multi-professional research team has all the necessary expertise to successfully complete each project stage, from system development to evaluation of effectiveness and cost utility, and implementation. The timeframe of this Forte application includes Objective 1 and the major part of the RRCT. RelevanceA decision support system that can now be developed in FRIDA has great potential to streamline Swedish multimodal specialist care
    AI methods in combination with “big-data” are recommended in precision medicine to optimize treatment. This AI support system can significantly improve health-related quality of life, reduce emotional suffering and sick leave for patients with chronic pain and be of great socio-economic value to society.

    Chronic kidney disease and cardiovascular disease cause enormous suffering for patients and high costs for society.  It has been suggested that the microbiome (bacteria and other microbes) in the gut and urine play an important role for the development of these diseases but community-based data are scarce. The overall aim of this proposal is to investigate the role of gut and urinary microbiome in the development of kidney and cardiovascular disease in a large community based cohort (SCAPIS) with detailed data on kidney and cardiovascular phenotypes.SCAPIS is the largest study to date with gut microbiome data analysed with state-of-the-art whole genome shotgun metagenomics (n=10,000). In addition, with this proposal I will carry out the first-ever urinary microbiome-wide association study in the community. Moreover, I will create a unique multi-omics database with microbiomics, metabolomics and proteomics analyses in faeces, blood and urine and adress our research questions with novel state-of-the-art advanced machine learning methods. Moreover,  will apply Mendelian Randomization analyses in order to establish causal associations that may help guide future experimental and intervention studies. If successful, the present research proposal may open up new horizons for research that in the end may lead to improved diagnosis, prognostication and treatments for patients with kidney or cardiovascular disease in clinical practice.
