Monica Susanna Brighenti
Immunologist studying immune responses and and disease mechanisms in tuberculosis, with focus on host–pathogen interactions and new treatment strategies.
About me
I am an Associate Professor in Immunology and Group Leader at the Center for Infectious Medicine (CIM), where I have been appointed since 2003. I completed my PhD at Lund University and Active Biotech Research (1997–2003) and was subsequently recruited to Karolinska Institutet to establish a new research platform focused on tuberculosis (TB).
With more than 23 years at KI, I have been actively involved in both research and education, including clinical and experimental studies, lecturing, graduate teaching, and research evaluation. I have extensive experience serving on boards and steering committees and I am frequently appointed to review doctoral education projects, grant applications, and scientific papers.
My overall ambition is to deliver high‑quality, modern teaching and conduct innovative, clinically relevant research, while contributing to strategic development within the academic community.
Research
My research focuses on understanding the immunopathogenesis of TB and developing host‑directed therapies for difficult‑to‑treat forms of the disease, including multidrug‑resistant TB. As part of this work, my group has established quantitative technologies for studying human TB using clinical patient samples and in vitro infection models in high‑containment laboratories, enabling pioneering studies of immune responses at the site of infection. Since then, we have made key discoveries on host–pathogen interactions and impaired immune functions in TB, providing the foundation for emerging immunomodulatory treatment strategies.

- Swedish Heart-Lung Foundation1 January 2026 - 31 December 2028Bakgrund: A life is lost to tuberculosis (TB) every 20 seconds, making it one of the world s deadliest infectious diseases. Lengthy antibiotic treatment contributes to resistance in Mycobacterium tuberculosis (Mtb) and fuels multidrug-resistant TB (MDR-TB). Host-directed therapies (HDTs) that enhance immune function represent an underexplored opportunity to improve patient s outcomes. While TB is typically treated with a one-size-fits-all approach, precision medicine requires stratifying patients by immune endotypes, defined by distinct pathophysiological features. Our previous work in Mtb models and clinical trials shows that histone deacetylase (HDAC) inhibitors can restore immune balance and improve treatment outcomes when combined with standard chemotherapy. Målsättning: This project aims to advance precision medicine for pulmonary TB by integrating clinical phenotypes with immune endotypes to predict prognosis and tailor HDTs using immunomodulatory compounds that enhance immune control and improve cure rates. Building on this concept, we are developing novel HDAC inhibitors with improved efficacy. Arbetsplan: We will use well-defined patient cohorts in Sweden (SWE-TB, n<50) and Ethiopia (ETH-TB, n=50), along with an in vitro infection model, to define immune endotypes responsive to HDAC inhibitors. First, clinical phenotypes and disease severity will be assessed using TB symptom scores, radiological findings, lung function, and peripheral inflammation markers. Next, deep immune profiling of myeloid and T cell responses in longitudinal blood and sputum samples will be performed using functional single-cell technologies. We will apply manual and data-driven approaches, such as clustering and dimensionality reduction, to identify immune endotypes and associate these with clinical phenotypes. We will also evaluate how different Mtb strain lineages influence host immunity and disease severity. Finally, selected HDAC inhibitors will be tested for their ability to enhance immune function and eliminate Mtb in patient-derived cells using high-content imaging, bacterial growth, and molecular assays. Betydelse: Combining findings on clinical phenotypes (Aim 1), immune endotypes (Aim 2), and Mtb genotypes (Aim 3), will guide chemotherapy decisions and assist identification of individuals most likely to benefit from HDTs (Aim 4). Precision medicine could modernize TB care by adding therapies that restore immune function in patients with distinct endotypes.
- Swedish Research Council1 December 2024 - 30 November 2027Multidrug-resistant tuberculosis (MDR-TB) is a serious global health problem resulting from inappropriate antibiotic treatment of pulmonary TB, which highlights the need for new drug regimens. In the project, Path-to-Resist-TB, we aim to join forces in the fight against antimicrobial resistance to increase the understanding of pathogenesis in patients with MDR-TB (Aims 1-2) and discover novel compounds with efficacy against drug-resistant mycobacteria (Aims 3-4). Cells and immune pathways in tissue biopsies from MDR-TB lungs will be studied using multiplex spatial protein profiling that allows for detailed visualization of host-pathogen interactions in diverse microenvironments with high resolution. We will also explore the diversity of the gut-lung microbiome and the effects on systemic inflammation after second-line MDR-TB therapy. In parallel, we will explore novel molecules with either direct anti-mycobacterial effects or indirect effects on host immunity. For this work, we will exploit a macrophage infection model and organotypic lung tissue model as well as relevant panels of disease-causing strains including MDR-TB and inherently drug-resistant non-tuberculous mycobacteria (NTM) using high-content imaging assays. The results will promote identification of pathogenic mechanisms and novel antimicrobial agents that will pave the way for development of alternative treatment options for difficult-to-treat TB patients and prevent the spread of drug-resistance in the society.
- Swedish Research Council1 January 2023 - 31 December 2026Multidrug-resistant tuberculosis (MDR-TB) is a serious threat to global elimination of TB and a difficult challenge in treatment and management of disease. This 4-year project aims to explore how immune checkpoint molecules are modulated in MDR-TB patients depending on clinical disease severity and bacterial strain virulence. Our original findings from TB patients show that antimicrobial macrophage and T cell responses are severely impaired, while myeloid-derived suppressor cells and regulatory T cells expand in the TB lesions. To advance our understanding of immunoregulation in MDR-TB compared to drug-susceptible TB, we will examine stimulatory and inhibitory checkpoint molecules in a well-defined patient cohort in Addis Ababa, Ethiopia. We will model disease progression and quantify disease severity at enrolment and after chemotherapy using a composite clinical TB score and chest X-ray grading. Drug susceptibility testing will determine the level of resistance and genotyping of patient´s strains will signify bacterial virulence. Longitudinal assessment of checkpoint molecules in peripheral blood and induced sputum samples involves deep profiling with RNA-sequencing, multicolor flow cytometry and in vitro suppression assays. We will also exploit in situ imaging of resected lung tissue from MDR-TB patients. This collaborative project offers untapped opportunities for translating new advances on checkpoint molecules in chronic infections into clinical applications for MDR-TB.
