Research
I am recognized in the global scientific community for my contributions to cancer research, particularly in the field of breast cancer. Since 2010, I have held the position of Professor of Cancer Epidemiology. I lead a dedicated research team focused on understanding the causes and consequences of breast cancer. Our success stems from our ability to effectively combine clinical, genetic, molecular, and questionnaire data with national registries and biobanking resources.
With over 400 scientific publications and an h-index of 71, my expertise spans epidemiology, clinical studies, and experimental research. This diverse experience supports my commitment to promoting effective interdisciplinary communication, a crucial skill in my recent roles as Co-Director of Cancer Research KI and as a member of prestigious organizations such as the Nobel Assembly, the Research Committee of the Swedish Cancer Society, and the Breast Cancer Association Consortium.
My team's research has significantly advanced the understanding of breast cancer, initially emphasizing the role of inherited genetic factors in determining risk and aggressiveness. More recently, we have explored genetic/molecular associations with breast cancer subtypes and treatment responses. We have also investigated the critical role of mammographic breast density/features in the diagnosis and prognosis of the disease.

- Swedish Research Council1 January 2026 - 31 December 2029Mammography screening reduces breast cancer mortality, yet interval cancers - tumors diagnosed between screening rounds - constitute a major limitation, often presenting with aggressive phenotypes and poor prognoses. This project aims to identify epidemiological, biological, and imaging determinants of interval breast cancer to enhance risk stratification and screening efficacy. We will investigate the association between inherited genetic susceptibility, circulating biomarkers, mammographic features, and hormonal/lifestyle factors with interval cancer risk. Additionally, we will assess prognostic differences between interval and screen-detected cancers and develop an improved risk model for tailored screening strategies.The study is based on extensive Swedish population-based cohorts with linked screening, clinical, molecular, and epidemiological data. We will employ next-generation sequencing to analyze rare genetic variants, assess blood-based biomarkers for early risk detection, and apply AI-driven mammographic analysis to identify imaging features associated with interval cancers. Statistical approaches, including Cox proportional hazards models and machine learning-based risk prediction, will be utilized.Our research will provide new insights into the etiology of interval cancers and their missed detection in screening. The results will inform risk-adaptive screening strategies, ultimately improving early cancer detection and reducing mortality.

Mammography screening is so far the only proven method that reduces mortality from breast cancer. Mortality from breast cancer is 20-40% lower in screened women compared to non-screened women.Among women attending the Swedish Mammography Screening Program approximately every third breast cancers is diagnosed with breast cancer in the interval between two screens, a so-called interval cancer. The interval cancers could be either masked by high mammographic density, missed at the previous screening round or fast growing and actually developing in between two screening rounds. Interval cancers are more common in women with dense breasts and are considered more aggressive than screen detected cancers.The risk of breast cancer and the possibility to detect a cancer differs profoundly between women. Women with mammographically dense breasts have an increased risk of breast cancer and the possibility to diagnose a prevalent breast is reduced due to “masking”. Women with dense breast are more likely to be diagnosed with an interval cancer then women with non-dense breasts.The aim of SMART, The Swedish Mammography Risk stratified Trial, is to test the effectiveness of individualised breast cancer screening. In short - does an individualised screening program identify cancers at an earlier, curable stage, compared to the current one-fits-all approach?
- Swedish Research Council1 January 2024 - 31 December 2027We will develop and use novel, biologically-motivated statistical models of tumor progression to elucidate mechanisms of breast cancer progression and to predict the risk of (in particular, aggressive) breast cancer. The models will be estimated using data from Swedish and Norwegian studies which represent some of the world’s most detailed population-based studies of breast cancer and breast cancer screening, combining longitudinal register, questionnaire, image and molecular data. The models are extensions of prior methods that we have developed that separate out the roles of factors in screening and symptomatic detection of breast cancer, and tumor onset, growth and spread. The information we create is vital for planning and evaluating approaches to (secondary) prevention of breast cancer. Our risk prediction models are novel, and rigorously incorporate screening information.We will also study in detail the background of false-positive mammography results – i.e. recalls that do not lead to a diagnosis, but can be a considerable psychological burden for women. Moreover, we will use simulation-based approaches to evaluate the performance of new (personalised) screening strategies.Finally, we will use our novel statistical approaches with detailed prescription data to study the efficacy of hormone therapy in estrogen receptor-positive patients.

- Swedish Cancer Society1 January 2023In Sweden, four women die of breast cancer every day. The long-term goal of our research is to reduce mortality from breast cancer through early detection of the disease and through improved identification of women at high risk of dying from the disease. In order to provide a comprehensive picture of factors associated with breast cancer prognosis, we will examine, in addition to traditional tumor factors, areas that have not been studied to the same extent, such as factors of importance for prognosis, such as hereditary genetic and mammographic characteristics. In this project, we will study genetic determinants of so-called interval cancer, an aggressive and deadly subgroup of breast cancer tumors that develop between two screening occasions. We will first investigate the importance of immune-related genetic variations in the development of interval cancer. Next, we will investigate the relationship between inherited genetics, adherence to hormonal treatment and prognostic outcome. Finally, we will investigate how mammographic breast density can be used to improve diagnostics and serve as a useful indicator of early response to hormone therapy. For the development of individualized screening of women at high risk for aggressive breast cancer, it is essential to understand the role of inherited genetics. Information about hereditary genotype will be able to be used by the doctors to optimize the hormonal treatment aimed at improved prognosis. Tracking changes in mammographic density can provide clinicians with more real-time information regarding treatment outcome than a "wait-and-see" approach can. Overall, we believe our work is important to advancing research in the field and ultimately saving or extending these women's lives.

- Swedish Cancer Society1 January 2020In Sweden, a woman is diagnosed with breast cancer every hour and every six hours a woman dies from the disease. Factors that are important for the prognosis are tumor characteristics, the patient's individual characteristics, how the patient responds to treatment and any complications. In other words, there are more factors than tumor characteristics that affect the prognosis of breast cancer. In previous studies, we have found that the patient's hereditary genetic background has a significant impact on the aggressiveness of the disease. We have also shown that breast density and other mammographic factors are crucial for proper diagnosis and prognosis. The overall aim is to study genetic and mammographic factors of importance for life-threatening breast cancer. The study material is taken from population-based studies on breast cancer, with biological samples, information from questionnaires and clinical data. Our specific purposes are to identify: 1) hereditary genetic factors 2) genetic abnormalities in tumors 3) mammographic density and mammographic characteristics and analyze the relationship to prognosis. As a prognostic model for fatal breast cancer, we use interval cancers as these are aggressive and clinically interesting because they are not detected during mammography examination. In the next few years, we anticipate that we have identified even more genes that can be related to life-threatening breast cancer. In addition, we have identified prognostic markers that we have validated in the prospective KARMA study. To improve the diagnosis and prognosis of breast cancer, we see that information about breast density will be included as an obvious part of clinical practice. Our long-term goal is to develop methods for individual screening and treatment by identifying women with an increased risk of life-threatening breast cancer, which in turn will be of great importance for survival.
