- Swedish Research Council1 December 2024 - 30 November 2027Heart failure is a significant health concern, often leading to repeated hospitalizations due to delayed diagnosis and inadequate self-care. The healthcare model is shifting from hospitals to community settings in many countries, where shortages of specialists are common. In these settings, patients are often elderly with multiple comorbidities, complicating the identification of worsening heart failure for healthcare professionals and impeding patients´ self-care at home. To address these challenges, we conducted studies on Remote Dielectric Sensing (ReDS) technology, which non-invasively measures lung fluid. In our feasibility study, ReDS proved user-friendly for primary care nurses, facilitating the identification of worsening heart failure and medication optimization. We developed a ReDS-guided heart failure management program with treatment algorithms and self-care support, led by primary care nurses. Currently, we´re conducting a pilot study of the program (Phase 1). In Phase 2, starting August 2024, we aim to assess its effectiveness on heart failure hospitalizations and cost-effectiveness in a randomized controlled trial. Simultaneously, we´ll evaluate program processes and assess implementation barriers and facilitators as part of Phase 3. Through this application, we seek funding for Phases 2 and 3. This project provides insights into a new community-based heart failure management model, which could mitigate hospitalizations and reduce healthcare costs.
