SMART2D, a multi-country research project funded by the Horizon 2020 Framework Programme of the European Union, that integrated 6 academic partners with Karolinska Institutet as the coordinating department. The projects aims waws to address the gaps in the care and management of type 2 diabetes mellitus (T2DM). The project ended in 2020.

There are gaps in the care and management of Type 2 diabetes mellitus (T2DM) worldwide - the inadequate identification of those at risk due to strong health center-based care, poor contextualization of evidence-based practices, and inadequate focus on vulnerable population groups.

Sharing of knowledge from an international project on how to manage your type-2 diabetes
In two socio-economically disadvantaged Stockholm suburbs, an urban township in South Africa and a rural area in Uganda, new methods are currently being tested to help people with type 2 diabetes take better control of their disease. The studies are part of the international research project Smart2d.
