  • From advanced kidney biopsy diagnostics to individualized treatment in IgA nephropathy

    IgA nephropathy (IgAN) is the most common form of glomerulonephritis worldwide and, given today’s limited treatment options, leads to dialysis‑requiring kidney failure in about 30% of cases. When Henoch–Schönlein purpura occurs concurrently, the condition is called IgA vasculitis with renal involvement (IgAVN). International research collaboration and newer analytical methods are contributing to an increasing understanding of the pathogenetic mechanisms in IgAN and the progression of chronic kidney disease in general. A number of more specific treatment approaches have been developed and are currently being tested in multicenter studies. One challenge is that IgAN is a heterogeneous disease, and patients with IgAVN are often excluded from the studies. Genetic variations and environmental factors can influence individual differences in disease course and treatment response. Our translational studies are based on archived biopsy tissue, longitudinally collected blood and urine samples, and clinical data from over 400 adult IgAN/VN patients who have been followed for up to 45 years from diagnosis. Disease controls are included at the time of biopsy and are followed up with new samples. Using advanced nano‑scale 3D imaging diagnostics, in combination with molecular analyses in tissue, blood, and urine, we investigate morphological changes and potential biomarkers that can help assess which treatment is likely to provide the greatest benefit for the individual patient at different stages of the disease course. The goal is to reduce progression to severe kidney failure and the associated cardiovascular disease and reduced quality of life.

  • Urinary biomarkers for early renal and cardiovascular disease in the Stockholm SCAPIS cohort (U-BIS)

    The SCAPIS study is a world-unique Swedish research study in the heart/lung area where 30,000 randomly selected people between 50 and 64 participated, in a collaboration between Gothenburg, Lund, Linköping, Uppsala and Umeå universities and Karolinska Institutet in Stockholm. Data collection from the study's baseline examinations was ongoing from 2013 to 2018. The aim of the study is to investigate the occurrence of cardiovascular, demographic and metabolic risk factors in relation to clinical and subclinical heart and lung disease. Blood and urine samples have been saved in Biobank. A re-examination of half of the individuals in SCAPIS 2 has begun, where 2500 of these are from Stockholm and will be examined at Danderyd Hospital. So far, no specific examinations have been performed in relation to the individuals' kidney health. Our project U-BIS is a substudy of SCAPIS 1 where we plan to analyze a range of potential urine biomarkers at baseline and their correlation to collected data regarding lifestyle factors, demographics and signs of cardiovascular disease. In the future, we also want to correlate these findings to the survey results in SCAPIS 2. Our analyses will be performed on saved samples from the 5000 research subjects recruited in Stockholm. New urine analyses are planned to be performed in the future with selected markers in specified subgroups of the research subjects from SCAPIS 2 when recruitment for that study is completed.
