    A large collaborate research project to address emerging healthcare challenges through innovation, cross-sector cooperation, and the integration of technological solutions. The Hospital@Home (H@H) initiative aims to break the singular silo approach by focusing on multiple conditions that represent high morbidity, often require intensive monitoring and coordinated care. The chosen use cases include cardiovascular, pulmonology, minor stroke/TIA, migraine, and cluster headache, as these use cases show large promise for improving prevention through early detection of disease progression or degeneration by using advanced monitoring tools. The total funding for H@H was 21,8 million Euro.
