Missing Link - Phase 2 workshops

The project is currently in phase 2, co-design of a new person-centred care transition from stroke unit to subsequent rehabilitation in the home. Persons who have had stroke, significant others and staff work together in workshops to develop the new care transitions.

The design process is performed in collaboration with OpenLab at KTH.
