Research Area

Much of the work in my research group focuses on chronic inflammatory diseases, mainly Rheumatoid Arthritis (RA).

About 0.7% of the adult population has RA; one in thirty individuals in the general population will receive a diagnosis of RA during his/her lifetime. The etiology is unknown but seems to include genetic susceptibility and environmental triggers, and to be markedly different between different subsets of patients with RA, suggesting that the clinical concept of "RA" harbours not one disease but several diseases, each with its distinct etiology.

Apart from RA, other chronic inflammatory diseases of interest include inflammatory bowel disease, ankylosing spondylitis and other spondyloarthropaties, and Wegeners granulomatosis. All these diseases are characterized by highly variable disease courses and response to treatments, and by the presence or gradual development of co-morbid conditions that may account for the major burden of disease or premature mortality in the long run. In RA, for instance, there is a markedly increased risk of cardiovascular disease.

Another common feature for many chronic inflammatory diseases is the recent possibility to use biological therapies (typically antibodies directed against key pathways in the immune system). Over the last decade, nine different biological drugs have been introduced against RA. These therapies, intended for chronic use as they do not cure but suppress disease activity, may be effective but are costly and have incompletely understood safety profiles.

The overarching aims of my research are therefore to study the descriptive epidemiology, etiology, co-morbidity, drug safety, and outcome of chronic inflammatory diseases. To do this, we use clinical epidemiological methods based on linkages of clinical and other registers of patients with these diseases, their treatments, diseases, and outcome. In particular, we use the clinical quality of care registers in Rheumatology.

My research group constitutes the clinical epidemiological node in a research network also including genetics and genetic epidemiology, immunology, clinical Rheumatology and other research groups at Karolinska Institutet, at other Swedish universities, and internationally.
