Structured information in a continuum of care for stroke patients
Continuation of the NI-STROKE-project.
The overall objective of this project is to improve quality of care and treatment results, leading to increased benefits for stroke patients through consistent, coordinated and coherent provision of information to all involved care providers and patients.
The current project consists of two subprojects:
- Subproject 1: Development of a structured epikrise template with automatic transmission to the National Stroke and SITS.
- Subproject 2: Development of structured journal templates that can be used in a continuous stroke care chain for documentation, communication and quality assurance.
Project start autumn 2011, planned final reporting December 2012.
The project is a collaboration between the BÄST-project (SLL), Strokeforskningsnätverket at SöS, Socialstyrelsen, HIC, Kvalitetsregistercentrum(QRC), Riks-Stroke, SITS etc.
